Fellowship Learning Experience

During this rotation, the Fellow would explore the critical role of digital health technologies in facilitating smooth transitions of care for patients. They would focus on understanding the challenges and opportunities in care transitions, such as hospital-to-home or hospital-to-long-term care facility transitions. The Fellow would learn how digital health tools can enhance communication, medication reconciliation, and care coordination during these transitions. They would gain hands-on experience in utilizing digital platforms, remote monitoring systems, and telehealth solutions to support seamless transitions and optimize patient outcomes.
